Begini Cara Klaim Asuransi Jasaraharja

JAKARTA, AKSIKATA.COM – Direktur Hubungan Kelembagaan Jasaraharja, Munadi Herlambang menjelaskan PT Jasaraharja akan mempercepat proses klaim jika diajukan pemudik.

“Syarat mendapatkan Asuransi Jasa Raharja dapat dilakukan dengan melengkapi berkas data diri dan korban tercatat di kantor polisi,” jelas Munadi, usai melepas keberangkatan Mudik Bersama BUMN di Jakarta, Selasa (18/4/2023).

Berikut Cara dan Syarat Mengajukan Asuransi Jasa Raharja:

1. Meminta surat keterangan kecelakaan dari unit Lakalantas Polres setempat atau instansi serupa yang memiliki wewenang (Misalnya: PT KAI untuk kereta api dan Syah Bandar untuk kapal laut).

2. Membawa surat keterangan kesehatan atau kematian dari rumah sakit

3. Membawa identitas pribadi korban (asli dan fotokopi), seperti: Kartu Keluarga (KK) Kartu Tanda Penduduk (KTP) Surat Nikah

4. Mengunjungi kantor Jasa Raharja dan mengisi formulir, diantaranya: Formulir pengajuan santunan, Formulir keterangan singkat kecelakaan, Formulir kesehatan korban Keterangan ahli waris jika korban meninggal dunia.

5. Menyerahkan formulir serta melampirkan dokumen pendukung kepada petugas

6. Untuk korban luka-luka yang mendapatkan perawatan harus memiliki: Laporan polisi berikut sketsa Tempat Kejadian Perkara (TKP) atau laporan kecelakaan pihak berwenang lainnya.

Kuitansi biaya perawatan, kuitansi obat-obatan yang asli dan sah yang dikeluarkan oleh rumah sakit; Fotokopi KTP korban Surat Kuasa dari korban kepada penerima santunan (bila dikuasakan) dilengkapi dengan fotokopi KTP korban penerima santunan; Fotokopi surat rujukan bila korban pindah ke rumah sakit lain.

7. Untuk korban luka-luka hingga mengalami cacat:

Laporan Polisi berikut sketsa TKP atau laporan kecelakaan pihak berwenang lainnya Keterangan cacat tetap dari dokter yang merawat korban, Fotokopi KTP korban, Foto diri yang menunjukkan kondisi cacat tetap.

8. Untuk korban luka-luka kemudian meninggal dunia: Laporan Polisi berikut sketsa TKP atau laporan kecelakaan pihak berwenang lainnya, Surat Kematian dari rumah sakit/Surat Kematian dari kelurahan, jika korban tidak dibawa ke rumah sakit, Fotokopi KTP korban dan ahli waris juga Fotokopi Kartu Keluarga (KK) Fotokopi surat nikah bagi korban yang telah menikah, Fotokopi akta kelahiran atau akta kenal lahir, bagi korban yang belum menikah, kuitansi asli dan sah biaya perawatan dan kuintansi obat-obatan,
Fotokopi surat rujukan bila korban pindah rawat ke rumah sakit lain.

9. Untuk korban meninggal dunia di TKP: Laporan Polisi berikut sketsa TKP atau laporan kecelakaan pihak berwenang lainnya;
Surat Kematian dari rumah sakit atau surat kematian dari kelurahan jika korban tidak dibawa ke rumah sakit; Fotokopi KTP korban dan ahli waris; Fotokopi KK Fotokopi surat nikah bagi korban yang telah menikah; Fotokopi akta kelahiran atau akte kenal lahir bagi korban yang belum menikah.

10. Menunggu proses pencairan
